Description
Use your clinical skills where they can open the door to care.
HONOR is seeking an experienced Licensed Clinical Social Worker (LCSW) to join our Safe Options Support (SOS) team serving Orange and Dutchess Counties. This role combines hands-on clinical work with the opportunity to provide clinical guidance and mentorship to other social workers while working alongside a collaborative outreach team.
You will meet individuals experiencing unsheltered homelessness where they are, build relationships over time, and help connect them with behavioral health care, housing, and ongoing community support.
Our approach centers each person's choices. The SOS team uses harm reduction, trauma-informed care, persistent engagement, and person-centered practices to support participants before and after they transition into housing.
What You'll Do
As the Senior Community Outreach Mental Health Clinician, you will:
Join the SOS team's outreach efforts by van and on foot throughout Orange and Dutchess Counties, including visits to encampments and other unsheltered locations.
Build trust with participants and assess behavioral health, physical health, housing, and social needs.
Conduct comprehensive psychosocial and clinical assessments and assist in developing appropriate care plans.
Provide crisis intervention, short-term therapeutic counseling, and clinical support before and after housing placement.
Coordinate care with hospitals, housing providers, community-based organizations, social service agencies, and other community partners.
Provide clinical guidance to the SOS team and supervision to assigned LMSWs, limited permit holders, or interns in accordance with New York State requirements.
Support participants in addressing barriers to care, including appointment navigation, service linkage, and coordination with providers.
Maintain timely and accurate documentation of participant encounters, services, care plans, and progress.
Collaborate closely with SOS leadership, Case Managers, peers, and other team members to support participant stability and successful transitions into housing.
Requirements
What You Bring
Master’s degree in social work.
Current and valid New York State LCSW license.
Three to five years of relevant clinical experience.
Strong clinical judgment in assessment, crisis intervention, and collaborative care.
The ability to provide clinical guidance and supervision to other social work professionals.
Strong communication, interpersonal, organizational, and documentation skills.
The ability to work effectively as part of a multidisciplinary team.
The ability to conduct field work, including walking over varied terrain and working in different weather conditions.
A valid New York State driver's license.
Experience working with individuals experiencing homelessness, community-based behavioral health services, or similar populations is preferred, but not required. Clinical supervisory experience is preferred.
What Field-Based Means at HONOR
This is a Monday through Friday clinical position that combines community outreach, direct clinical practice, care coordination, documentation, and team collaboration.
Field work takes place throughout Orange and Dutchess Counties and may include outreach by vehicle and on foot at encampments and other community locations. You will work as part of the SOS team rather than independently navigating this work without team support.
The position requires regular travel within the service area and the ability to work in varying community environments and weather conditions.
The position follows a Monday through Friday schedule, with occasional flexibility outside of regular business hours based on participant or program needs.
Why Join HONOR?
This is an opportunity to use your clinical expertise beyond the traditional office setting.
As part of the SOS team, you will help individuals experiencing unsheltered homelessness build connections to care, navigate complex systems, transition into housing, and establish greater stability in the community. You will also have the opportunity to bring your clinical experience to the broader team through guidance, collaboration, and mentorship.
If you are looking for a Monday through Friday clinical role that blends direct practice, community-based care, team collaboration, and clinical leadership, we would like to hear from you.
